Clarity’s Columbus based services will unite into a cohesive Care Hub at one convenient location, purchasing property located at 3203 Middle Road, Columbus IN.

Connectivity of Care
Renovations and initial expenses of building project. Clarity will integrate services from other providers to better address the complex set of social, economic, and healthcare factors that influence health and sexual behaviors.

Establish a New Model of Care
Two years of operating expenses for all locations with $3.5M allocated annually. The Columbus Hub will be a pilot model that can be replicated in high need areas, helping develop a comprehensive and integrated care approach.

- Expand building space from less than 9,000 square feet to 18,000 square feet
- Provide an established location with more than 100 parking spaces
- Connect us to a well-known medical facility in a medical park area that lends credibility
- Give improved accessibility from the city bus line
Connecting Care
Integration of Services
At both the STI testing and treatment clinic and care center, we meet sexually at-risk patients in need of care beyond their initial visit. Often, a client may benefit from a referral to the Hearts Restored program, a mental health care professional, or a community service provider. To improve overall health outcomes for its patients, we are taking the initiative to connect Clarity’s services under the same roof with other care providers.
By integrating services from other providers, Clarity can better address the complex social, economic, and healthcare factors influencing health and sexual behaviors. Collaboration between healthcare providers, educators, and community organizations will help organize and deliver healthcare services to ensure continuity, coordination, and integration across different types of care.
By connecting care in one location, we’re removing barriers that prevent individuals from obtaining the care they need to flourish.

This new approach to holistic sexual health care is designed to prevent gaps in care, reduce the likelihood of fragmented or disjointed services, produce better health outcomes, and improve patient experiences.
It prioritizes individual needs, preferences, and values while recognizing the importance of involving patients in decision-making and planning at each step.

Clarity is leading the way in advancing a new model of care to address the root causes behind behaviors that put adults and students at risk of having or causing unwanted pregnancies.
Better outcomes for sexual health occur when an individual’s psychological, relational, cultural, spiritual, physical, and emotional needs are addressed. Connectivity of care will improve individual health outcomes by increasing access to comprehensive sexual health care.
Clarity’s new model of care will connect:
More women, men, and teens who have experienced sexual trauma with Hearts Restored Care Groups to discover healing and healthier living.
Expecting mothers to prenatal care and providers to improve health outcomes for moms and their babies.
Individuals to mainstream health providers for specialized care needs to improve physical and mental health outcomes.
Resources and information sharing between Clarity's STI testing clinic, pregnancy care center, educational program, and Hearts Restored.
Expecting moms with material resources needed to reduce the financial burden associated with pregnancy and parenting.
Individuals with Clarity professionals who are establishing a culture where every life is valued and celebrated as a gift from God.
